Making a Difference

​The Local Registrar of each Ohio county is responsible for maintaining the birth and death records of county residents. The Registrar's office is located within the Division of Vital Statistics. Certified Birth and Death Certificates can be obtained at the Adams County Health Department between the hours of 8:00 A.M to Noon, and 1:00 P.M. to 4:30 P.M., Monday through Friday, or by mailing a completed application and $25.00 fee to:  ​

Certified Birth Certificates for births from any Ohio county can be obtained from the Adams County Health Department. Photo identification is required.
